To help expedite the business-end of our services, we have published the forms and contracts we regularly use. Please note that the documents presented here are regularly updated as the business and legal needs arise. While these forms represent our usual business practices, the version used at your contract closing might be different.

License of Electronic Rights
This form is used permit the use of images not created by the LightWurx photographers in a LightWurx developed website. Any images given to us for such use must have an accompanying form signed by the original photographer and the client - models, in most cases, but not exclusively.

Property Release Form A
This form is for releasing property (including artwork, catalog goods, etc.) for imaging. LightWurx owns the image copyrights, but the use of them is exclusively for a designated website.

Property Release Form B
Property Release Form B is used for granting permission to photograph images exclusively for the use of LightWurx Media and is not specifically identifying the target goals. The end use may include advertisements. While the form has a location for money exchanges, this form may be used for "TFP" purposes as well by indicating "Zero Dollars".

Photographic Assignment-Estimate-Invoice Form
This form is used for a variety of paid shoot options. This form can be used in many LightWurx engagements: note that this form can be "Assignment", "Estimate" or "Invoice". Be sure the appropriate box is checked identifying your use.

Stock Photography Invoice
This form is used for the licensing of an image used in print (books, flyers, brochures, etc.) as well as web-based release.

Model & Performer TFP/TFCD Form
If we are exchanging services (photographer and model, giving their time and talents without monetary compensation), we will use this form. While this form is traditionally used for fashion and glamour photo shoots, we have also used this form photographing bands (in that case, each band member will have to sign their own form).
